21 NCAC 21 .0106 FORMS

Forms used by the Board, and available from the Board office upon request, include the following:

(1) Application for Licensing as a Geologist,
(2) Application for License by Comity,
(3) Application for Renewal of License,
(4) Application for Reinstatement of License,
(5) Application for Corporate Registration, and
(6) Certificate of Registration.

21 NCAC 21 .0107 FEES

(a) Completed application forms must be accompanied by the prescribed fee. Application fees will not be refunded regardless of Board approval or disapproval of the application. Prescribed fees shall be not more than:
 

(1)  application forms for licensing as a geologist, including a copy of the Geologists Licensing Act and Rules:
$  5.00
(2) application for license: $ 50.00
(3)  examination: $350.00
(4) application for license by comity:  $ 50.00
(5) biennial renewal of license:  $ 70.00
(6) replacement of license:  $ 10.00
(7) application for reinstatement of license:  $ 50.00
(8) licensed geologist stamp and cost plus  $   5.00
(9) application for corporate registration:  $ 50.00
(10) annual renewal of certificate of registration for corporations  $ 25.00
(11) registered geological corporation stamp cost plus:  $   5.00
  
(b) All licenses will expire biennially on July 1. Biennial renewal fees received after July 1 and before September 1 of the year due shall be subject to the assessment of a late payment penalty of twenty-five dollars ($25.00). All certificates of registration for corporations will expire annually on July 1. Annual renewal fees for certificates of registration for corporations received after August 1 and before December 31 shall be subject to a late payment penalty of twenty-five dollars ($25.00).
(c) Licenses that have not been renewed by September 1st may only be renewed by filing a reinstatement application and submitting a reinstatement fee.
(d) Licenses that have been suspended under G.S. 89E19 may be reinstated by filing a reinstatement application and paying the reinstatement fee.

SECTION .0200 RECORDS AND REPORTS OF BOARD: RETENTION AND DISPOSITION

21 NCAC 21 .0201 RECORDS OF BOARD PROCEEDINGS

21 NCAC 21 .0202 RECORDS OF APPLICATIONS

All records of applications for registration shall be retained by the Board.

21 NCAC 21 .0203 ROSTER OF LICENSED GEOLOGISTS

Every individual licensee has the continuing responsibility of keeping the Board currently advised of the licensee's proper and current mailing address and the name or names under which the licensee is practicing. Each licensee shall immediately notify the Board of any and all changes of association or address.

21 NCAC 21 .0204 APPLICABILITY OF BOARD RULES

SECTION .0300 LICENSING OF GEOLOGISTS

21 NCAC 21 .0301 REQUIREMENTS FOR LICENSING

(a) Education. In determining whether an applicant meets the minimum education requirements of the Geologists Licensing Act, the Board is authorized to determine which colleges and universities are properly accredited, and which geologic science courses qualify for meeting the minimum requirements.
(b) References. Five letters of reference submitted to the Board which shall satisfy the Board as to the character, reputation, responsibility, integrity and competence of the applicant. These letters of reference must be submitted by licensed or qualified geologists or professional engineers. No member of the Board shall act as a reference for any applicant for licensing. At least two of the five letters of reference must be submitted by licensed or qualified geologists who are familiar with the applicant's work in the field of geology.
